Heart to Heart Hospice - - Responsibilities: Provide direct patient care under RN supervision according to the Hospice Aide/Homemaker Plan of Care; Assist with safe transfers and ambulation per agency policy; Report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care; Practice accepted infection control principles; Promote positive, supportive, respectful communication to patient/family and other employees
