HCS Service Coordinator

In this role, you'll partner with each person and their program provider to connect them with the medical, social, educational, and community resources they need. You'll play a key role in shaping their quality of life through thoughtful assessment, service planning, ongoing monitoring, and proactive crisis prevention.

What You'll Do:

• Support individuals in achieving their unique goals
• Coordinate access to essential services and supports
• Build collaborative relationships with program providers and community resources
• Monitor progress and ensure services align with each person's needs
• Help prevent and manage crises with compassion and professionalism

What You Need:

• Bachelor's degree in a human services field (MUST UPLOAD TRANSCRIPT WITH YOUR APPLICATION)
