Overview

The 4Kids4Families Care Coordinator is responsible for the treatment, coordination and monitoring of services to highly acute children and youth. Works with network providers to ensure trauma-informed, evidence informed services are being provided.

Responsibilities

Other duties may be assigned as necessary
  • Responsible for utilization management of children and youth on their caseload.
  • Facilitates placements, oversees service delivery, and coordinates discharge planning to ensure children/youth are being served in the least restrictive environment.
  • Ensures that services provided to children and families by 4Kids4Families staff are provided with fidelity and in accordance with approved models and curriculum.
  • Identifies and develops treatment recommendations for clients.
  • Refers and directs clinical services to clients to include but not limited to therapeutic services and psychiatric services.
  • Provide consultation for foster parents, adoptive parents, biological parents, respite provider, childcare staff, and other caregivers, as indicated.
  • Maintain accurate, timely, and complete case notes, billing documentation, treatment plans, progress notes and other documentation according to licensing, accreditation, and agency standards.
  • Attends trainings and meetings as required.
  • Participates in case planning conferences, Professional Consulting Team, School meetings, court hearings, staffing, etc., as appropriate.
  • Maintains regular contact with family and child and monitors psychological, mental, emotional, physical, and environmental well-being through phone contacts, email, and home visits.
  • Provides crisis intervention as needed to prevent escalation of high-risk behavior, relapse, hospitalization, placement disruption, and other negative outcomes on a 24-hour on-call basis.
  • Consults with medical doctors and other specialists, and/or treatment team members concerning treatment plans and amends plans as needed.
  • Maintain child and/or family records in 4Kid's electronic client information system utilizing assigned 4Kids technology resources. These resources may include, but are not limited to desktop computers, laptop computers, touchscreen devices, and scanners.
  • Review client files on a quarterly basis to access service plan implementation, progress towards desired outcomes, and future appropriateness of service plan.
  • Maintain a manageable caseload to ensure outcomes and practice requirements are met while taking into consideration the level of care of clients served.
  • Participates in the auditing and monitoring of service providers according to the terms of their contract.
  • This position is subject to providing direct supervision to youth that are placed by the SSCC. At times, they may be required to provide supervision to a youth that has not had

    placement located, and therefore, serve in a caregiver capacity until a licensed placement is found.
  • Perform all duties within contract, state and federal regulations, company policy, and regulatory agency standards.
  • May be required to drive vehicles, be insurable with our auto insurance provider, and complete company vehicle training.

Qualifications

Bachelor's Degree from accredited college or university required; master's degree preferred. At least three (3) to five (5) years of experience working with children in the child welfare industry.
